Special Education Records

The following are records you will need to keep to advocate for your student:

  1. IEPs (and 504 plans)

  2. Prior Written Notices (PWNs)

  3. Behavior Intervention Plans (BIPs) and Functional Behavior Assessments (FBAs)

  4. All evaluations and reevaluations

  5. All private evaluations

  6. A list of medical providers and therapists, and their contact information, that work with your child

  7. All emails with the school and school district (save them in a folder)

  8. IEP progress reports

  9. Report cards

  10. Standardized test reports

  11. Discipline notices

  12. Restraint and Isolation reports

Often times the paperwork given to parents is in draft form, and the final versions are kept in the student’s school file. In order to request your student’s school records, contact your school district’s special education department and ask for a copy of your child’s special education file. You can request it in electronic format if that is more convenient for you. Here is the state regulation regarding special education records.

Your child’s school file (including grades and discipline records) should be kept at the school, and is called a “cumulative file.” You should be able to request them from the front desk or your child’s principal.
