Requesting a Special Education Evaluation
Make sure to write your request in an email or letter to the school principal. If you are sending a letter, make sure to date it, and send it in a way to prove the date it was sent (stamped by the school front desk, fax, or certified letter). If he or she does not respond, contact your school district’s special education department right away. In order to request a special education evaluation, it is helpful to be aware of the regulations in Washington state. You are not required to use a particular form, but you may request one from your school district’s special education department.
In the letter, state your child’s disability or be specific about why you suspect your child may have a disability. Give specific examples of problems your child is having at school and why you think your child may need special education services. Keep in mind that a special education evaluation covers more than just grades or academic problems; it includes a wide variety of developmental needs.
The school is not required to evaluate on your request, but there is a procedure that must be followed. You will be provided a consent form to evaluate. Make sure you sign and date the form, and ask for a copy for your records. You may be asked to sign a medical records release form to obtain records relating to your child’s disability.
